Local Participation Form

The Local Participation Form tracks total student participation in each of six Reflections Program arts categories, and also as a percentage of total students served (enrollment) by each local PTA/PTSA unit. This information is used to track growth of the Reflections Program, and its accuracy is critical to obtaining and reporting on grants and sponsorships that support the program.

The Local PTA/PTSA Participation Form Excel Document includes two tabs, located at the bottom of the form. The first tab opens a Local Contact Information page. Please complete all information on this page. Please do not use abbreviations.

The following tab opens a page for local participation input. Simply complete all information for each arts category that applies to your local unit. Please do not leave fields blank. For example, if your unit does not participate in a particular arts category, do not leave the field blank. Rather, insert a zero.

Upon completion of this form, please save a copy for your records. then save the form on the DVD you send to your next level up. Local Participation Form

The Local Participation Excel Form is a required form for all participating PTA/PTSA units. After completing the form, please email a copy to Reflections@pta.org and to your State Reflections Chair.

Local and Area Master Entrants Form

The Winning Master Entrants Form is the Master Record of all winning Reflections entries. This information is used to recognize all Reflections winners and their local PTA units, meaning that it will be used in press releases, certificates, award letters, and trophies. It is also the information used to contact student winners and PTA units, and to return award winning photography and visual arts entries to their owners. Please make every effort to assure accuracy of this information. Please proof all entries for accurate spellings of student names and be sure to provide the full local PTA/PTSA unit name and PTA or PTSA identification. Please do not use any abbreviations.

Upon completion of this form, please save a copy for your records, then save a copy to CD or DVD and send it to the next level up in your state system. Please see the State Arts Chair Guide for further instructions.

State Participation Form
