The Area Participation Form tracks total local, council, district, and regional Reflections Program participation (as applicable in each state), and also as a percentage of total participation by area PTA/PTSA units located in each state. This information is used to track growth of the Reflections Program, and its accuracy is critical to obtaining and reporting on grants and sponsorships that support the program.

The Area Participation Form Excel Document includes two tabs, located at the bottom of the form. The first tab opens an Area Contact Information page. Please complete all information on this page. Please do not use abbreviations.

The following tab opens a page for area participation input. Simply complete all information for each arts area that applies to your area unit. Please do not leave fields blank. For example, if your area does not participate in a particular arts category, do not leave the field blank. Rather, insert a zero.

Upon completion of this form, please save a copy for your records, and then save the form on a CD or DVD and mail it to the next level up in your state PTA system.
