A participation form is required from all council and district/region PTAs participating in the PTA Reflections Program.

National PTA requires each state to track the number of PTAs that participated and the total number of works entered in the state’s Reflections Program. The participation forms make that possible and are vital to understanding how the Reflections Program shifts and grows each year.

Use the appropriate participation form below to record the number of entries judged by your PTA and the PTAs you serve, as well as the number of entries forwarded to the next level of judging. Please provide all of the information requested, including contact information. Then, submit the form to the next level of judging along with winning artwork.
